Server Reboots with Panic on I/O to Pool

Description

The server consistently reboots on kernel panic. "I/O to pool .. appears to be hung on vdev guid.
We have 8 iscsi initiators connecting to 8 targets.
Can I find out where or what this vdev guid is trying to access? Also is there any other information I can supply?

Attached are the last textdump and info files from /data/crash.

panic: I/O to pool 'Site2Recorder' appears to be hung on vdev guid 16133963651444927299 at '/dev/gptid/6eed833f-5ca1-11e5-ae7d-e4115bda7dc4'

Pool is a single 12.5T vdev, which is a vdisk on an HP/COMPAQ RAID controller.

Sounds like the RAID controller is going out to lunch, and ZFS is correctly detecting that I/O is hung and shooting the system down. I'd check the event log on the RAID controller and see what errors (if any) it contains.

There really isn't a FreeNAS bug here to fix.
